Original job description

Your Opportunity:

South Health Campus (SHC) is guided by four foundational pillars: Collaborative Practice, Wellness, Patient and Family-Centred Care, and Innovation. These principles shape the delivery of care and services across all programs. The Allied Health Department provides services to both inpatient units and outpatient programs. This 0.4 FTE Hand Therapist (Occupational Therapist or Physiotherapist) position is based in the SHC Hand Clinic, a fast-paced, client-centred environment that values collaboration, innovation, and excellence in care. Working within a primary therapist model and under the general supervision of the Allied Health Assistant Manager, the successful candidate will provide assessment and treatment for patients referred to the outpatient Hand Clinic in accordance with professional practice standards. The ability to demonstrate accountability, collaborate effectively, and maintain a continuous learning mindset is essential. Strong skills in communication, self-management and initiative are vital to success in this role. In addition to direct patient care, the role includes consultation and education within the Allied Health Department, as well as participation in program development initiatives. This position is physically demanding and requires repetitive lifting, prolonged postures, and transferring of patients and equipment. The caseload for all positions within the Allied Health department may be subject to change based on operational considerations.

As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant stakeholders and supports within the client's physical, institutional, cultural, and social environments. You will include clients and families as integral members of the healthcare team, encouraging their active participation in all aspects of care. You will collaborate with clients and families to determine, implement, and evaluate rehabilitation plans. You will participate in a variety of interprofessional teams to build awareness, enhance supports, and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for clients. You will contribute to quality improvement and the professional community and participate in peer mentoring and student supervision.
