Your Opportunity:
The RN Interventionist position, with the Adult Team in Home Care, is responsible for assessing, planning, organizing, implementing, reviewing and evaluating the needs of shorter term clients (Acute and Rehabilitation clients) aged 18 – 64 and their caregivers/families requiring community based services. The RN Interventionist fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings. The RN Interventionist is also responsible for providing direct nursing interventions to a select group of Long Term Supportive, Maintenance and End-of-life clients.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
Additional Required Qualifications:
3-5 years recent nursing experience with acute care focus. Ability to function well in a fast paced, challenging environment. Ability to work independently using innovative approaches to case manage a diverse client caseload. Demonstrated clinical assessment skills, ability to provide direct nursing interventions including HPTP, wound care, management of tubes lines, drains, medication management, mobility and transfers, interdisciplinary coordination. Excellent interpersonal, communication skills, organizational and time management skills. Demonstrated critical thinking and decision making skills. Demonstrated ability to operate within an electronic health record. Proven ability to work in a multidisciplinary team. Good negotiation and conflict resolution skills. Knowledge of ethical principles and their application in the clinical/home setting. Physically able to perform all assigned tasks. This role requires travel; possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes as a requirement of this position. You will be required to operate a laptop computer and mobile phone, and for handling (lifting, lowering, carrying, pushing, and pulling) as a regular part of the job. For example: lifting approximately 30 pounds in an out of a vehicle on a regular basis and carrying this weight up and downstairs throughout the day is common within this job. Intermediate level of computer skills.
Preferred Qualifications:
1-2 years experience in community based setting. Experience with mental health and/or addictions/harm reduction, palliative care, chronic disease management.
