Union: CUPE Number of vacancies: 1 New or Replacement Position: New Site: Toronto Rehabilitation Institute – University Centre Department: Brain Program – Stroke 9 South Reports to: Manager, Program Services Wage range: $38.01 - $39.02 per hour (start rate in accordance with collective agreement) Hours: 18.75 hours per week Shifts: 8hrs – Day/Evening/Night & 12hrs - Day/Night - Weekday/weekend Status: Permanent Part-Time As a member of a multi-disciplinary Team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.Provide education to patients and families regarding relevant deficits and related health promotion interventions Works effectively independently as well as collaboratively with inter-professional team Contributes to the care planning process and follows a designed plan of care Provides safe, competent care within Scope of Practice Actively participates in education/mentorship of RPN students and new staff Actively participates in professional, program and hospital-wide initiatives Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patientsCurrent certificate of competence from the College of Nurses of Ontario (General Certificate) and member in good standing, required Diploma preparation as an RPN in a recognized education program for Nursing preferred Successful completion of Basic Cardiac Life Support (C level) Heart Saver Plus (Canada) required Ability to demonstrate strong health assessment skills Current medication certificate (if not included in diploma program) required Medical asepsis (if not included in diploma program) preferred Recent experience in rehabilitation nursing care of neurological patient populations required Experience providing care to patients who require trach care, suctioning, tube feeding and bowel and bladder management required Familiarity with principles of rehabilitation and practice implications required Ability to assess and care for patients with multiple medical needs Demonstrated ability to identify, assess, and monitor clinical status and intervene appropriately with changes in clinical status, required Ability to appropriately care for patients who present with severe behavioural dysfunction, including engagement and re-direction, required Ability to recognize changes in behaviour and follow prescribed behavioural management strategies required Ability to appropriately care for patients who present with severe cognitive and physical impairment required Knowledge of standards of practice and applicable health care legislation Computer proficiency required or willing to pursue appropriate computer trainingWhy Join UHN: In addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks. Responsibilities

Provide safe, competent, compassionate nursing care to patients in the Brain Program, including neurological rehabilitation, assessment, monitoring, and appropriate intervention when clinical status changes. Educate patients and families, contribute to care planning, collaborate with the interprofessional team, support students and new staff, and participate in program and hospital initiatives.

Requirements

Requires current registration in good standing with the College of Nurses of Ontario, Basic Cardiac Life Support certification, and recent experience caring for neurological rehabilitation patients, including patients requiring tracheostomy care, suctioning, tube feeding, and bowel and bladder management. Candidates must demonstrate strong assessment and behavioural management abilities; an RPN diploma, medication certificate, medical asepsis training, and computer proficiency are preferred or required as specified.
