Bayshore HealthCare is one of the Canada’s leading providers of home and community health care services and is a privately owned company. Bayshore HealthCare is proud to showcase its achievement as a Platinum member of Canada’s Best Managed Companies Program every year since 2006. Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings

At Bayshore Integrated Care Solutions (ICS), we're transforming the transition from hospital to home for older adults across Ontario. Through a large and growing network of Hospital-to-Home programs, our interdisciplinary care teams deliver innovative bundled care pathways designed to improve outcomes, reduce readmissions, and provide a seamless experience for clients and families.

Position Type: Independent Contractor

Working within one interdisciplinary team and one shared client record, you'll have visibility into the entire client journey and the opportunity to see the impact of your interventions throughout recovery.

You'll be responsible for the assessment, treatment, education, and ongoing management of clients requiring Occupational Therapy services, helping older adults maximize independence, safely transition home, and achieve meaningful outcomes in their communities.

Maintain accurate and timely clinical documentation in accordance with the standards of the College of Occupational Therapists of Ontario.
Provide clinical guidance and direction to support personnel involved in the delivery of Occupational Therapy services, as appropriate.
Participate in case conferences, care planning discussions, and collaborative decision-making processes.

Education & Registration
