Calgary Homeless Foundation (CHF)

Calgary Homeless Foundation guides the fight against homelessness. Fueled by this purpose, we envision the day when homelessness is rare, brief, and non-recurring - perhaps an episode in someone's life, but never a condition that defines it. Working in alignment with governments and collaboratively with service providers and community partners, we help translate complex system needs into coordinated, efficient ground-level action that maximizes the impact of every resource and creates lasting pathways out of homelessness.

The Position

CHF has a strategic commitment to provide governance and operational supports towards a recovery-oriented, coordinated system of care. The system of care is comprised of services to those who are experiencing homelessness (outreach, emergency shelters, and hidden homelessness), those who are seeking supportive housing (Coordinated Access and Assessment), and over 45+ programs providing housing supports. The position provides daily operational management of the Coordinated Access and Assessment (CAA) data quality and communication functions, including pre-placement data quality checks, weekly placement committee supports, CAA inbox monitoring, and post-placement data entry. This role is also responsible for coordinating and chairing the weekly placement committee meetings, providing support to train new Housing Strategists, and supporting current Housing Strategists with mentoring. and data quality

Accountability and Deliverables

As the CAA Coordinator, you will be responsible for daily operations of CAA, placement committees, Housing strategist support, CAA data and communications. You will report to the Manager, CAA, and work closely as part of the team with CHF System Planners, HMIS Support Specialists, and Data Analysts.

The following list describes your responsibilities:

Data Quality and Data Entry

Perform daily "Eyes on the List" data quality checks for the Coordinated Entry Tool and CAA
Follow-up with Housing Strategists to correct any concerns which act as a barrier to effective referrals.
Make corrections directly in the Homeless Management Information System (HMIS) as appropriate.
Record referrals in HMIS and manage the appropriate data access transitions.

CAA Coordination

Chair weekly placement committee meetings to ensure referrals are being made into adult, youth, and family supportive housing programs related programs within the Homeless Serving System of Care {HSSC)
Liaise with and support more than 240+ housing strategists who administer the Coordinated Entry Tool (CET) assessment and who currently operate within multiple organizations across Calgary.
Providing CET/CAA mentorship opportunities and data quality support for active Housing Strategists at various door agencies in Calgary.
Provide support to the Manager, CAA to provide Coordinated Entry Tool (CET) training to new Housing Strategists.

Communications and Support

Manage the inflow of housing program communications for open spaces and referral status.
Collate program communication information for utilization during weekly placement committee meetings.
Participate in and support placement committee meetings, taking notes and supporting any data-related needs
Manage the CAA inbox and respond to requests and inquiries related to CAA and the Coordinated Entry Tool
Support with the coordination of CET training and CAA related events (ex., community consultations).

Team Participation

Work directly with the System Planning team to coordinate with housing programs and support with referrals.
Be attentive to patterns and work with data analysts and HMIS support specialists to surface and test hypotheses and monitor CAA trends
Participate in CHF-wide planning and implementation team processes.

Education and Experience

The ideal candidate will have the following qualifications:

2+ years direct experience with CAA/HMIS and 2+ years in data quality and data entry roles,
Strong acumen for mindfully performing detail-oriented, repetitive work
Strong learning capacity (new databases, new processes, new systems)
Strong capacity for communication and relationship-building
Trauma-informed practice awareness is an asset
