Position Summary:

The Client Support Worker provides administrative support functions and maintains various office systems and procedures related to the ID Clinic and supporting the Community Health Programs Team.

The Client Support Worker serves as a champion of the Hamilton Urban Core’s mission and vision and works in a manner that is consistent with the Centre’s values, beliefs, core competencies and commitment to quality in service. The Client Support Worker is an essential member of the client-centered, inter-professional, multidisciplinary team of health providers at Hamilton Urban Core. The Client Support Worker will work with other members of the Centre’s teams to effectively and efficiently serve clients and communities using responsive and innovative approaches to services and service delivery framed by health equity and the social determinants of health.

Accountability:

The Client Support Worker (ID Clinic) reports to the Community Health Promotion Manager. Overall accountability is to the Executive Director.

Key Responsibilities:

Responsibilities are divided into two primary categories:

ID Clinic (70% or 3.5 Days)

Provide administrative and clerical support to the ID Clinic
Maintain up-to-date systems for client files and information,
Maintain sound office procedures and systems, by designing, implementing and maintaining effective internal control tools and/or processes that are relevant to the ID Clinic.
Maintain an effective administrative filing system and monitor procedures for storing data and ensuring its safety.

Administration (30% or 1.5 Days)

Assist with the Centre’s client intake/referral process through the HUCCHC website administrative dashboard
Facilitate meetings of the ID Clinic and Community Health Programs Team by scheduling meeting space, making the appropriate arrangements, preparing meeting materials and overseeing details related to the meeting as required
Prepare meeting and committee minutes, reports, record keeping, data collection, and other information as required.
Prepare internal and external correspondence such as memo, letters, health promotion monthly calendar, reports, newsletters, and other administrative tasks as may be assigned

Other:

Actively participate in the Centre’s quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as Team, Case Conference and Staff meetings
Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning
Maintain and develop professional competence.
Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework.
Work as part of the interprofessional team at the Centre and adhere to the Centre’s core competencies and the CHC Model of Care
Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core

Performance Targets:

The approximate number of clients served will be 8 to 10 per day based on an average 7.5-hour workday for a 1.0 full time Client and Administrative Support Person. The average appointment length will be 30 minutes with the exception of instances where a cultural interpreter is engaged or when administrative or documentation procedures require more time.

Qualifications:

Relevant education and experience working in a fast-paced, health and/or community-based setting
Experience in general office procedures and/or business administration
Demonstrated experience maintaining a high level of efficiency through administrative systems and practices
Excellent organizational skills, high degree of accuracy in work and ability to work independently
Knowledge and proficiency in the use of computers and various software applications
Good written and spoken communication skills and problem-solving abilities
Knowledge of and sensitivity to the barriers and issues impacting on people and their families or supports

Schedule:

Monday to Friday from 9:00 am - 5:00 pm/ 12:00 pm to 8:00 pm/ Occasional Saturdays
