Fraser Health invites you to join an innovative and unique primary health care team located in Langley – a fast-growing urban centre surrounded by parkland, in the heart of the Lower Mainland.
Our team provides urgent care after hours and on weekends, with a complement of 2.6 FTEs primary care providers (family physicians and/or nurse practitioners). Duties will initially focus on urgent care, with longitudinal care added in the future.
We operate in a team-based care model that includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. You will participate as a full and active member in the delivery of team based primary care services. Together, we are dedicated to extending intentional and coordinated care to all members of the community, including the vulnerable and medically complex. Join us to provide timely access to patients, families and caregivers to comprehensive, person-centred, culturally safe and quality care.

Responsibilities
Provide urgent and longitudinal primary care as part of a multidisciplinary team in Langley. Collaborate with physicians, nurse practitioners, and allied health professionals to deliver comprehensive, person-centred care to the community.
Requirements
Must be a qualified family physician or nurse practitioner capable of working in a team-based care model. Experience or interest in providing culturally safe care to diverse and medically complex populations is required.
