Primary Location- Montague Patient Medical Home
The primary care model is evolving to team-based patient and family centered care. Team-based care occurs when a number of health providers with different professional backgrounds work together with patients, families, caregivers and communities to deliver care so that the right care is provided at the right time by the right provider. Team-based care will be delivered from Patient Medical Homes in Patient Medical Neighbourhoods, a concept developed and endorsed by the College of Family Physicians of Canada (CFPC). A Patient Medical Home is “…a family practice defined by its patients as the place they feel most comfortable - most at home - to present and discuss their personal and family health and medical concerns...”. A Patient Medical Neighbourhood is a network of care involving all health care providers and services outside of the Patient Medical Home. Patient Medical Homes coordinate and integrate access to services within the Neighbourhood and community for care of the patient.
This position works directly in consultation with the client and team to provide comprehensive client assessment and treatment ensuring effective and efficient delivery of primary care for illness, injury or disability affecting the musculoskeletal, cardiopulmonary, vascular, and neurological systems. The incumbent will address chronic disease and other physical health problems with this patient medical home population. As an integral member of the patient medical home team, the physiotherapist will work with physicians, nurse practitioners, registered nurses, licensed practical nurses and other allied health in providing comprehensive primary care services. The role is an expert leadership role acting as a professional physiotherapy resource with clients and the team.
Duties will include:
- Provide physiotherapy assessment, consultation, analysis (including diagnosis and prognosis within professional scope), and treatment, including utilization of and recommendation/ clinical prescription of specialized equipment, based on best practice evidence.
- Record all aspects of the client’s assessment and treatment including initial assessment, client goals, treatment plans, progress notes and transfer /discharge notes.
- Participate in and/or lead case conferences and collaborate in care planning activities with other health care professionals, families and/or community partners.
- Evaluate/monitor/manage client care plans (including discharge plans) and client program.
- Educate clients, caregivers, staff with regards to the client treatment plan, e.g. preventative measures and rehabilitation strategies.
- Provide outcome measures and professional opinions/recommendations with respect to a client’s condition.
- Maintains a culture of safety within the organization and will be aware of their role in supporting client safety within the organization.
- Remains aware of their role in supporting patient safety within the organization.
- Remains up to date and current with department and facility policies and processes that support client safety.
- Develop, implement, coordinate and lead primary health care physiotherapy service including jurisdictional scan of other programs/services regarding delivery model, program parameters and collecting, monitoring, evaluating and reporting data to ensure quality assurance.
- Collaborate with other programs or service areas to identify new evidence-based practices and develop, implement and evaluate program materials. E.g. collaborate with primary care and chronic disease programs and community-based services.
- Other Duties as required.
Minimum Qualifications:
- Must have successfully completed a Master’s Degree in Physiotherapy or equivalent;
- Must be eligible for licencing with the PEI College of Physiotherapists;
- Must have considerable experience as a practicing physiotherapist with a minimum of one year in the area of musculoskeletal injuries (public or private out-patient musculoskeletal experience). Preference given for community level physiotherapy experience;
- Demonstrated ability to work independently within professional expertise and apply sound clinical decision making;
- Demonstrated ability to work cooperatively and collaboratively in an interdisciplinary team environment;
- Strong communication skills, both written and verbal;
- Effective time management and organizational skills;
- Demonstrated commitment to continuous professional development; Must have additional training in orthopedic physiotherapy beyond entry level;
- Proficiency working in a computerized environment;
- Ability to develop and lead training sessions for clients and staff;
- Travel is required; Must have a valid driver’s license and access to reliable means of transportation;
- Demonstrated leadership, interpersonal, facilitation and collaboration skills;

Salary Range: $39.93- $48.99 per hour
Bi-Weekly Hours: 75 hours Bi-weekly / Mon-Fri 8-4 /Days/ Evenings and Weekends as Required
