Patient Medical Homes coordinate and integrate access to services within the Neighbourhood and community for care of the patient.

Provide outcome measures and professional opinions/recommendations with respect to a client’s condition.
Maintains a culture of safety within the organization and will be aware of their role in supporting client safety within the organization.
Remains aware of their role in supporting patient safety within the organization.
Remains up to date and current with department and facility policies and processes that support client safety.
Develop, implement, coordinate and lead primary health care physiotherapy service including jurisdictional scan of other programs/services regarding delivery model, program parameters and collecting, monitoring, evaluating and reporting data to ensure quality assurance.
