KEY FUNCTION:

Support families of infants/children from birth to school entry who are at risk of, or have a known developmental delay. Conduct home-visit services throughout the Greater Sudbury and District’s catchment area.

REPORTING:

Under the general direction of the CQI Manager, Infant and Child Development Services and the Children’s Treatment Centre.

DUTIES:

Conduct documented home visits to provide guidance to families and regularly monitor their infant/child’s development.
Provide support to families within areas of concern affecting their infant/child (e.g. muscle tone, vision, feeding, communication, parent-child attachment, play skills, etc.).
Communicate sensitively and respectfully with families.
Assess and monitor the infant/child’s development using informal and formal screening tools.
Develop a Family Service Plan to provide parents with recommendations on their infant/child’s development between home visits.
Provide resources and make referrals to community resources for additional services specific to an individual infant/child.
Assist families in their applications for appropriate special needs funding.
Assist families plan their infant/child’s entry into daycare, school or other programs.
Participate in staff committees as required.
Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
Participate in hospital training as required.
Assist with improvement initiatives as required.
Perform other duties as required.

QUALIFICATIONS

EDUCATION AND TRAINING:

Minimum of a two (2) year Diploma in Social Services, Education, or Health Care with a focus on child development from an accredited college.
Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

EXPERIENCE:

Minimum of one (1) year full-time equivalent experience working with infants and preschool children with developmental or physical disabilities or those at risk of delay.
Direct experience working with parents of infants and young children or adults in the provision of developmental intervention and caregiver training is required.
Direct experience in screening, assessing, interviewing and administering developmental programming and support is required.
Experience providing services to multi-need families from varying socio-economic backgrounds is preferred.
One (1) year experience working in-home visiting/intervention program is preferred.

KNOWLEDGE/SKILLS/ABILITIES:

Demonstrated knowledge of infant and child development in the context of prematurity, developmental delay, genetic and medical conditions.
Demonstrated knowledge around approaches to working with families to support their child’s development within their daily routines.
Demonstrated knowledge of children’s services/organizations, resources, and programs in the community for children with developmental disabilities.
Demonstrated ability to work sensitively within different cultural groups.
Demonstrated ability to consult with multi-disciplinary professionals, patients, physicians, volunteers and the public to provide optimum service to children.
Demonstrated computer skills with intermediate proficiency in Microsoft Office Software (i.e. Word, Excel, Power Point and Outlook) and applicable patient information systems.
Demonstrated working knowledge of equipment used in the delivery of client care.
Demonstrated interpersonal and communication skills.
Ability to prioritize tasks.
Demonstrated compliance with HSN’s privacy policy.
Demonstrated commitment to the safety of co-workers and patients.

PERSONAL SUITABILITY:

Successful Vulnerable Sector Check is required and renewal every three (3) years.
Ability to travel between local sites.
Valid driver’s license with access to vehicle may be required.
Willingness to travel occasionally within Northern Ontario.
Ability to use tact and discretion in dealing with health care providers, patients/clients and families.
Proven ability to work with a high degree of independence to prioritize duties.
Demonstrated commitment to ongoing professional development.
Demonstrated professionalism in dealing with confidential and sensitive issues.
Demonstrated positive work record and good attendance record.
Ability to meet the physical and sensory demands of the job.
