Community Healthcaring KW is currently recruiting for the following position:
REGISTERED PRACTICAL NURSE
$54,519.14 - $65,685.62 per annum
Permanent, Full-Time FTE 1.0
37.5 hours a week
The Primary Care Registered Practical Nurse provides direct primary care within their full scope of practice and participates in the development, implementation, monitoring and evaluation of services for individuals, families, and the community. The Primary Care Registered Practical Nurse will also support to the Primary Care Connection Clinic, Refugee Health, and other services within the CHC teams.
Responsibilities:
- Complete brief client histories, set up equipment in room for client visits .
- Triage client’s needs, either by telephone or in person.
- Assess and screen clients with minor episodic illnesses according to established protocols.
- Perform necessary clinical functions to facilitate delivery of comprehensive primary care and patient flow (e.g., assist providers with prescription ordering; clean equipment).
- Perform basic nursing functions (e.g., dressing of wounds, injections, suture /clip removal and assisting with other procedures as requested).
- Provides Health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Practical Nurses and all relevant provincial legislation and regulations.
- Perform non-clinical duties such as inventory counts, supply orders, and distribution.
- Keep accurate health records and up to date clinical notes in accordance with College of Nurses Standards and Community Healthcaring K-W policies.
- Attend and participate in case conferences for client services coordination.
- Assist with the planning, implementation and evaluation of other health promotion/disease prevention programs and services for individuals and client populations as determined by CHKW.
- Stock exam rooms sufficiently and consistently.
- Maintain contact with pharmaceutical company representatives and maintain a current inventory of pharmaceutical samples.
- Organize printed resources such as client education materials, diagnostic and other requisitions.
- Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
- Assist in the development of operating budgets for assigned projects/programs and monitor expenditures.
- Provide orientation, ongoing support, coaching and monitoring as a Student Supervisor, to meet the requirements of the Health Centre and of the educational institution.
- Provide support, training, monitoring and on-going communication as a Volunteer Supervisor as needed for the volunteer to successfully fulfill their position description.
- Maintain a valid driver’s license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with the Health Centre’s policies.
- Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener as determined by the Health Centre. A minimum of one evening per week will be required.
- Participate in team and staff meetings, other meetings, and committee work as appropriate to support organizational goals and service delivery.
- Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
- Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
- Work in a manner that preserves privacy and confidentiality.
- Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.

Accountability
This position reports to: Manager, Primary Care
Qualifications:
- Current registration as a Registered Practical Nurse in good standing with the College of Nurses of Ontario.
- Two or more years’ employment experience as a Registered Practical Nurse, including in a primary care or community setting. A combination of primary care, community and acute care experience will be considered.
- Knowledge of available resources accessible to clients (e.g. food banks, local shelters, mental services) an asset.
- Current and ongoing CPR certification, ideally through Heart & Stroke.
- Demonstrated ability to communicate effectively in English, both written and verbally.
- Additional language(s) an asset.
- Competency in the use of Microsoft Office Suite.
- Experience with electronic health records and other databases is an asset.
- Ability to work independently as well as an effective member of an interdisciplinary team.
- Proven ability to work collaboratively.
- Ability to maintain strict confidentiality.
- Experience working sensitively with culturally and economically diverse populations.
- Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
- 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
