Home Instead is a trusted leader in providing personalized, non-medical home care and support to older adults and their families. We are committed to enhancing the quality of life of seniors by delivering compassionate, reliable, and individualized care.

We are seeking a dedicated and relationship-focused Client Care Coordinator to join our team in Vancouver. This role is ideal for someone who combines strong client service and communication skills with a genuine passion for supporting older adults and their families.

As a Client Care Coordinator, you will play an important role in ensuring our clients receive the highest quality of care. You will work closely with clients, families, CAREGivers, healthcare professionals, and community partners to assess needs, develop individualized care plans, coordinate services, and maintain strong ongoing relationships.

This is a full-time position offering $60,000 annually, plus benefits, with opportunities for professional growth in a collaborative and supportive work environment.

Primary Responsibilities

Conduct client and family consultations to understand care needs, preferences, and goals.
Complete client assessments and develop individualized care plans.
Introduce and match CAREGivers with clients based on care needs, preferences, and compatibility.
Conduct regular quality assurance visits and follow up with clients and families to ensure satisfaction and quality of care.
Monitor and update care plans as client needs change.
Maintain accurate and timely client documentation, assessments, care plans, and reports.
Build and maintain strong working relationships with clients, families, CAREGivers, healthcare professionals, and community partners.
Collaborate with medical professionals and other service providers involved in a client's care.
Respond promptly and professionally to client and family concerns and coordinate appropriate follow-up.
Support the development of strong community relationships and promote Home Instead services to healthcare and community organizations.
Organize, prioritize, and coordinate multiple responsibilities in a fast-paced environment.
Communicate effectively with individuals at all levels, both verbally and in writing.
Work independently while contributing actively to a collaborative team environment.
Participate in presentations, community outreach, and relationship-building activities as required.

What You Bring to the Role

A valid driver's license and reliable access to transportation.

Experience & Qualifications

Experience or knowledge in the following areas is considered an asset:

Home care, healthcare, social services, or senior care.
Working with older adults and their families.
Alzheimer's disease and other forms of dementia.
Client assessments, care planning, or case management.
Nursing, social work, or another healthcare-related discipline.
Customer service, sales, relationship management, or business development.

A background in nursing (LPN), social work, healthcare, home care, or a related field is strongly preferred. Equivalent combinations of education and relevant professional experience will also be considered.
