Humber River Family Health Team (HRFHT)
Job Title: Manager, Primary Care Programs and Clinical Operations
Primary Portfolio: Progress Place, I Help Centre, Across Boundaries
Location: Multiple community-based locations in Toronto Compensation: $77,673 annually Benefits: Comprehensive benefits package (health, dental, vision, EFAP) and HOOPP defined benefit pension plan Schedule: 5 days a week, Monday to Friday Start Date: As soon as possible
About the Organization
The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centred primary care to optimize individual and family health within the community.
We provide comprehensive primary health care, including chronic disease management, health promotion, and disease prevention, to a rostered population of patients in the northwest sub-region of the Greater Toronto Area. Our interdisciplinary team of healthcare professionals works collaboratively to deliver high-quality, patient-centred, respectful, and inclusive care.
About the Position
Humber River Family Health Team is seeking a collaborative and experienced Manager, Primary Care Programs and Clinical Operations to lead the day-to-day operations of its assigned community-based primary care service locations. The successful candidate will oversee interdisciplinary staff, patient access, service delivery, and operational performance while working as an equal management partner to advance programs and quality improvement across the organization.
What You Will Do
- Lead operational planning, workflows, patient access, existing waitlist processes, referral triaging, and service coordination across Progress Place, I Help Centre, and Across Boundaries.
- Supervise, coach, schedule, and support assigned Interprofessional Health Providers and other designated staff, including recruitment, onboarding, performance reviews, and employee relations.
- Work collaboratively with the other manager on program development, implementation, evaluation, quality improvement, clinical service standardization, and community partnerships.
- Address clinical practice concerns and support complex care coordination in consultation with Lead Physicians and appropriately authorized clinicians.
- Coordinate staff training, interdisciplinary meetings, outreach activities, and clinical initiatives, including vaccine clinic activities where applicable.
- Support medical directive review and implementation in partnership with physician and clinical leadership.
- Monitor access, service delivery, staff capacity, risks, and performance; prepare operational reports and contribute to organization-wide planning.
- Support health and safety, infection prevention and control, privacy, and safe, respectful workplace practices.
What You Bring
- Postsecondary education in health, social services, management, or a related field; relevant professional registration is an asset.
- Progressive experience in primary care, healthcare, or community-based service operations, with demonstrated staff supervision and interdisciplinary leadership.
- Understanding of patient access, referral systems, clinical operations, quality improvement, privacy, and workplace safety.
- Excellent communication, problem-solving, conflict resolution, relationship building, and organizational skills.
- Ability to work across multiple locations and collaborate effectively with physicians, staff, patients, and community partners.
Reporting to: Executive Director
Management Partnership
Both managers report directly to the Executive Director and hold equivalent management authority within their assigned portfolios. Program oversight is collaborative across the organization. The established centralized waitlist and triaging system is an existing operational process, not a new system development assignment. The other manager oversees Weston, Dixon, Keele & Finch, and Wilson.
