About This Position
Make a meaningful impact in the lives of patients and families by providing comprehensive psychosocial care within a collaborative healthcare environment. As a Social Worker, you will support patients and families through illness, trauma, grief, bereavement, and life transitions while helping them navigate complex healthcare and community systems.
In this role, you will conduct psychosocial assessments, provide therapeutic counselling, develop care and discharge plans, and coordinate services with interdisciplinary teams and community partners. You will also provide consultation and education to healthcare professionals, patients, and families on topics such as adaptation to illness and disability, trauma, substance use, bereavement, community resources, and risk management. Working closely with hospital and community agencies, you will help ensure seamless transitions of care and optimal patient outcomes.
Beyond direct clinical practice, you will contribute to program development, quality improvement initiatives, research activities, professional education, and interdisciplinary collaboration to strengthen services and address emerging community needs.
