Join the Cancer Programs Section in making a meaningful impact on women’s health across Minnesota. In this role, you’ll be a key part of the Sage/SagePlus Patient Navigation team - helping women access life-saving cancer screenings and supportive services.

You’ll connect directly with patients by phone, answering incoming calls, educating eligible women about SagePlus, and guiding them to participating clinics. You’ll also conduct outgoing calls to actively reach women in the community, schedule clinic appointments, and ensure they feel supported every step of the way.

Beyond navigation, you’ll work one-on-one with SagePlus patients to encourage healthy lifestyle changes and provide follow-up health coaching using Motivational Interviewing techniques. This is a meaningful blend of outreach, education, and direct patient support - perfect for someone who is passionate about helping others access care and build healthier futures.

Primary Responsibilities Include

Planning and coordinating Sage/SagePlus participant navigation to support program completion at partner clinic and/or community sites.
Communication Liaison between program staff and clinical/community partners.
Recruit underserved populations to Sage/SagePlus services, schedule screening appointments and effectively respond to all callers’ inquiries.
Provide Health Coaching for SagePlus participants using Motivational Interviewing techniques.

In accordance with HR/LR Policy #1422 Telework, this position is eligible for up to 50% telework for applicants who live in Minnesota or in a bordering state within 50 miles of the assigned work location. Candidates residing outside of Minnesota further than 50 miles of the assigned work location at the time of application must be able to relocate to Minnesota within 30 days of their start date. The telework policy may exempt employees who live more than 50 miles from their primary work location from the 50% in-office requirement, depending on job requirements, helping the State of Minnesota access top talent across the state.

This is a temporary position with an anticipated end date of 09/29/2028.

Minimum Qualifications

One (1) year of experience providing client or patient support in a health, social services, or community-based setting. Experience must include each of the following:

Experience communicating clearly and professionally with diverse individuals by phone, email, and in person.
Experience using computer systems for documentation, data entry, and scheduling (e.g., electronic medical records, appointment systems, phone scripts).
Experience organizing and prioritizing multiple tasks while maintaining accurate records and attention to detail.
Experience applying public health or health-related knowledge, such as cancer screening, heart health, or healthy behavior support

Required

A degree in Public Health, Health Promotion, Community Health, Psychology, or another related degree may substitute for the required experience as follows:

A doctorate may substitute for twenty-four (24) months of experience.
A master’s degree or JD may substitute for eighteen (18) months of experience.
A bachelor’s degree may substitute for one (1) year of experience.
An associate’s degree may substitute for six (6) months of experience.

Minimum Qualifications To Be Assessed At Interview

Ability to communicate clearly, compassionately, and professionally with diverse individuals through phone, email, and in-person interactions.
Ability to prioritize multiple tasks, manage workflow efficiently, and maintain accurate, detailed documentation.
Ability to assess participant needs, make appropriate decisions independently, and problem-solve when navigating complex situations.
Ability to work effectively with clinical partners, community organizations, and internal team members to support patient navigation and program completion.
Ability to adjust to changing program needs, support events in various settings, and manage occasional shifts in schedule or priorities.

Preferred Qualifications

Experience using Motivational Interviewing techniques in a health or community-based setting to support behavior change and patient engagement.
Prior experience with public health programs, particularly those focused on cancer screening, heart health, or chronic disease prevention.
Demonstrated success in patient navigation or care coordination, including working with clinical partners, community organizations, or underserved populations.
Proficiency with electronic medical records or data systems, with a strong ability to document accurately, manage program data, and maintain compliance with reporting requirements.
Bilingual proficiency, especially in Spanish, to support communication with diverse Minnesotan communities.

Additional Requirements

This position requires successful completion of a background check and employment reference check.
