About the Opportunity
Reporting to the Manager, Clinical Network (Integrated Acute and Episodic Care) and working closely with the Operations Manager(s) at the zone level; the Cardiovascular and Stroke Coordinator will facilitate the development and dissemination of provincial standards and service delivery models and knowledge translation related to provincial cardiovascular and stroke care at the local site, zonal and province wide level as applicable. The coordinator’s work spans the continuum of care to ensure improvement in health and the care of Nova Scotians with a particular focus on cardiovascular health, coronary syndromes, heart failure and transient ischemic attack and stroke.
In alignment with the Integrated Acute and Episodic Care Network priorities and in collaboration with local teams, the coordinator will facilitate provincial activities within their local area, zone, and province wide as applicable. The coordinator facilitates the update of standards, protocols, and service delivery models through work with local committees and teams. The coordinator will assist with development of protocols, tools, and policies to facilitate integrated cardiovascular and stroke care that is appropriate and based on best practices. The coordinator will coordinate and facilitate network focused priorities and activities at the local site level such as implementation of standards, guidelines, processes and audits.
The Cardiovascular Coordinator receives direction from the Integrated Acute and Episodic Care Network related to integrated cardiovascular health and stroke care and works collaboratively with local site clinical and operational leads and staff to support implementation and monitoring of local and provincial quality improvement initiatives.
About You
We would love to hear from you if you have the following:
- Graduate from an approved School of Nursing
- Registered or eligible for registration with Nova Scotia Nursing and Midwifery Regulator (NSNMR)
- Minimum of (3) three years recent clinical experience working with stroke and cardiovascular patients
- Demonstrated experience in program development, facilitation, and evaluation
- Computer proficiency (e.g., MS Office suite, email, and internet use)
- Proven ability to build collaborative relationships
- Experience in change management
- Demonstrated analytical thinker, self-directed and an innovator
- Competencies in other languages an asset; French preferred

Hours
- Long assignment full-time position; 75 hours biweekly
- Approximately 18 months
Compensation and Benefits
$41.58 - $51.11 hourly
$52.90 hourly with 25 yrs RN experience
