Job Summary
The Home support worker is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the client’s needs.

Responsibilities
Prepare meals, perform light housekeeping, provide personal care and companionship, and assist clients with outings, shopping, errands, medication reminders, ambulation, and transportation. Follow each client’s care plan and complete required documentation and charting under the supervision of the Care Coordinator.
Requirements
Applicants must have a current clean background check or be willing to obtain one, and a negative TB skin test or chest X-ray. They must be able to travel within the service territory and have access to the internet and a mobile phone.
