About the Role
Are you a high-performing professional who loves building relationships and wants to make a real impact in your community? Right at Home Maple Ridge is seeking a growth-oriented Care Planner & Community Relations professional.
This role has two core areas of responsibility: business development and community relationship building, and care planning and client intake. You will work across Maple Ridge, Pitt Meadows, and surrounding areas, helping grow community partnerships while supporting families through the care planning process.
Business development in this role is relationship-driven, with a focus on building trusted community partnerships rather than transactional sales. You will connect with local doctors, hospital discharge planners, medical clinics, and community groups to ensure families get the support they need.
Whether your background is in professional sales, hospitality management, or the healthcare industry (such as an HCA, LPN, or Medical Office Assistant looking for a career growth path), we are looking for a confident relationship builder who is comfortable developing new community connections. If you are outgoing, motivated by achieving results and contributing to the growth of the organization, and possess the empathy required to guide families through care onboarding, this role is for you.
