Company Overview

At Right at Home Canada, we pride ourselves in offering a full range of exceptional support and care services for those with physical, medical, or memory impairment, at home, wherever home is to them. Our flexible, professional and relationship-oriented support is there when our clients need it most.

About the Role

Are you a high-performing professional who loves building relationships and wants to make a real impact in your community? Right at Home Maple Ridge is seeking a growth-oriented Care Planner & Community Relations professional.

This role has two core areas of responsibility: business development and community relationship building, and care planning and client intake. You will work across Maple Ridge, Pitt Meadows, and surrounding areas, helping grow community partnerships while supporting families through the care planning process.

Business development in this role is relationship-driven, with a focus on building trusted community partnerships rather than transactional sales. You will connect with local doctors, hospital discharge planners, medical clinics, and community groups to ensure families get the support they need.

Whether your background is in professional sales, hospitality management, or the healthcare industry (such as an HCA, LPN, or Medical Office Assistant looking for a career growth path), we are looking for a confident relationship builder who is comfortable developing new community connections. If you are outgoing, motivated by achieving results and contributing to the growth of the organization, and possess the empathy required to guide families through care onboarding, this role is for you.

Core ResponsibilitiesBusiness Development & Community Relations
  • Local Networking: Connect daily with healthcare professionals, medical clinics, and community groups to position us as their trusted partner.
  • Referral Management: Build authentic, long-term relationships with local influencers to generate incoming client inquiries.
  • Educational Presentations: Lead one-on-one meetings and group presentations to educate community sources on our specialized services.
  • Brand Ambassadorship: Represent our organization at professional networks, wellness fairs, trade shows, and community events.
  • Pipeline Tracking: Maintain consistent records of outreach activities and relationship progress using our internal platform.
Care Planning & Client Intake
  • Inquiry Guidance: Respond to inbound inquiries from families, acting as a compassionate guide through their care options.
  • Consultations: Conduct detailed client assessments and personal home consultations to establish trust and safety plans.
  • Care Plan Design: Develop customized care paths and collaborate with internal scheduling teams to launch services.
  • Client Advocacy: Serve as an ongoing resource for families, updating their personalized care plans as their medical or social needs evolve.
Who You Are
  • A Relationship Builder: You are confident initiating new connections, building trust, and developing long-term referral partnerships.
  • Goal-Oriented: You are self-motivated, accountable, and comfortable working toward defined goals, KPIs, and growth targets.
  • An Exceptional Communicator: You are comfortable presenting to professional groups and speaking with medical professionals or anxious family members alike.
  • Empathetic & Organized: You can easily pivot from a community networking meeting to a sensitive, compassionate discovery call with a family in need.
Required Qualifications
  • Experience: Minimum 2 years of experience in healthcare, home care, care coordination, client services, business development, or a related field.
  • Required Competencies: Ability to balance client-centered care planning with relationship development, community engagement, and business growth initiatives.
  • Mobility: Valid Driver’s License, a clean driving record, and access to a reliable vehicle for daily territory travel.
  • Background Check: Clear Vulnerable Sector Check (VSC) issued within the past 6 months.
  • Certifications: Current First Aid and CPR Level C certification.
  • Legal Status: Legally entitled to work in Canada.
What We Offer
  • Competitive base salary of $55,000–$65,000 per year.
  • Performance-based commission and bonus opportunities that recognize business growth and results.
  • Predictable full-time hours without the burnout of traditional shift work.
  • A supportive, team-oriented culture with feedback and coaching.
