SE Health is Hiring Personal Support Workers (PSWs) - Journey Home Hospice

Most Canadians want to die at home, but where do you go to die when you don’t have a home? This is a question that Journey Home Hospice – operated by Saint Elizabeth Foundation – is answering for patients experiencing homelessness and structural vulnerability. With 13-beds in the heart of downtown Toronto, four beds in Windsor, and an international, teaching and learning program, we are creating specialized trauma-informed, culturally safe, and harm-reducing approaches to care that are innovating care options for people in need in Canada and beyond. Requirements

Current PSW Certificate mandatory and availability to work flexible shifts (Days 7am-7pm)
Direct experience working with Mental Health patients and/or homeless populations preferred
Hospice Palliative care experience and ongoing continuing education is an asset
Experience working with vulnerable populations
Excellent Communication skills both orally and verbally along with exceptional computer skills
Well-developed interpersonal skills and excellent organizational skills
Able to work independently as well as part of an integrated team (nurse, physician, hospices, etc)
Effective problem-solving and conflict management skills
A current CPR and Standard First Aid certificate is required

Responsibilities

Provide specialized trauma-informed and harm-reducing care to patients experiencing homelessness and structural vulnerability within a hospice setting. Work collaboratively within an integrated team of nurses, physicians, and other hospice staff to deliver excellent patient care.

Requirements

Must hold a current Personal Support Worker (PSW) certificate and a valid CPR/Standard First Aid certificate. Experience working with mental health patients, homeless populations, or in hospice palliative care is preferred.
