Job Description
Windsor shelters are often unable to provide planned palliative care deaths in their facilities and homeless individuals are likely to lack the family and social support networks that assist in providing care in the community. As a result, these individuals are dying on the street and in hospitals. Through the partnership with Assisted Living Southwestern Ontario (ALSO) and the City of Windsor, we now have a 4 bed cluster of residential hospice beds. Windsor satellite site provides accessible, equitable end-of-life care for people facing barriers to traditional services, including those experiencing homelessness or marginalization. Care is evidence-based, harm-reduction focused, trauma-informed, and culturally responsive, honouring each person’s dignity, lived experience, values, and needs.

Requirements
- Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse (RPN)
- LEAP Certificate Required / Willing to complete LEAP Certificate within 6 months of hire
- Minimum 1 year Hospice Palliative care experience and ongoing continuing education is preferred
- Experience working with vulnerable populations
- Mental Health Nursing Certification or equivalent training preferred
- 1 year Mental Health experience and ongoing continuing education is an asset
- Well-developed interpersonal skills and excellent organizational skills
- Able to work independently as well as part of an integrated health care team (PSWs, RN, Physician, Nurses, Hospices Volunteers)
- Effective problem-solving and conflict management skills
- Works well in a changing environment with an appreciation for continuous quality improvement processes
- A current First Aid and CPR certificate is required
- Availability to work 12 hour shifts
