Position Summary:
Reporting to the Director of Care/Director of Nursing or designate, the role of the Registered Practical Nurse encompasses a wide range of professional nursing responsibilities in accordance with the Standards of Practice as outlined by the College of Nurses of Ontario. Under the direction of a Registered Nurse (RN), the Registered Practical Nurse (RPN) provides professional nursing care to residents and oversees the care given by non-registered staff in accordance with individual resident care plans and in a manner consistent with the College of Nurses of Ontario Standards of Practice, the regulations set out in applicable legislation and the policies and procedures of the Home.

Qualifications:
- Current Certificate of Competence from the College of Nurses of Ontario.
- Gerontology Certificate preferred.
- Knowledge and expertise in Resident Assessment Instrument (RAI-LTCF).
- Comprehensive knowledge of nursing and health care practices, as well as knowledge in Infection Control Practices.
- Effective human relation skills and demonstrated commitment to working with seniors.
- Excellent communication skills and the ability to work independently and collaboratively are essential.
· Ability to read, write and follow written instructions in English.
Hours of Work:
- Available 7 days/week per Collective Agreement
- Flexible hours per Collective Agreement
Main Duties and Responsibilities:
- Performs duties of the registered practical nurse as per job routines.
- Participates as required in the formulation and review of policies and procedures, goals and objectives.
· Orders, receives and administers prescribed medications and treatments in accordance with the standards set by the College of Nurses of Ontario.
· Observes and monitors conditions of residents and reports changes in physical or mental status.
· Contributes to the development, implementation and evaluation of resident care plans.
· Assists Team members with admitting, discharging and transferring residents.
· Updates non-registered staff on physician/nurse practitioner orders, care changes, home updates, etc.
· Assists in maintenance of resident records and ensures that all required documentation including medication and treatment charting is completed.
· Performs security checks when on night shift.
· Performs responsibilities of the position with the legislative and regulatory standards set out in applicable Provincial legislation, and the policies and procedures of the Home.
· Assists with care to residents including lifting and transferring, personal care, feeding, emotional support and other.
· Works co-operatively with staff, volunteers, service providers and other constituents to meet resident and family needs.
· Participates in and/or leads assigned committees and teams.
· Performs other duties as assigned.

- Works cooperatively with students and volunteers to enhance resident care and foster community linkages.
- Participates in the orientation process for residents, students, volunteers, and staff.
Care Coordination
- Collaborates with the interdisciplinary team to achieve positive resident outcomes.
- Effectively participates in conferences with the interdisciplinary team.
- Contributes to positive customer services with residents, families/Substitute Decision Makers (SDMs) and visitors in day-to-day work.
Occupational Health and Safety
- Complies with the legislative, regulatory and internal responsibilities as set forth in the Occupational Health and Safety Act, including health and safety, Workplace Hazardous Material Information System (WHMIS), Workplace Violence, Fitness for Duty etc.
- Advises supervisor/designate immediately of all staff incidents that may result in an Occupational Accident Claim, any concerns with the physical plant, incidents or injuries and completes required forms.
- Ensures a safe environment for residents, visitors, and staff.

Quality Management
- Participates in all aspects of Quality and Risk Management as applies to the nursing area.
- Participates in the collection of clinical indicators in collaboration with the interdisciplinary team.
- Contributes to the development of recommendations for corrective actions to improve residents’ quality of life and reduce/mitigate risk.
