General Description
The Suboxone Treatment Coordinator reports to the Mental Wellness Coordinator and is responsible for a wide range of case management activities and programming offered for people engaged in treatment for opioid use disorder in the community of Sipekne’katik. Working within a multi-disciplinary team, the Suboxone Coordinator will act as a liaison between community agencies and the Sipekne’katik Opioid Treatment and Recovery Program, facilitating treatment planning, case management, and delivering targeted interventions across a broad spectrum of health determinants to ensure comprehensive quality of care. A sound working understanding of community health approaches in First Nations communities and various community-based agencies in Sipekne’katik and throughout Nova Scotia will be required.
Responsibilities
Individual Treatment and Management plan 70%
- Meet new clients for an orientation session to review programming options, assess the needs of the individual, and review safe practices/harm reduction strategies in the context of treatment for opioid use disorder.
- Develop Individualized treatment plans with clients, including emotional, spiritual, physical, and mental well-being
- Support clients seeking opportunities to make progress on their wellness goals through engaging with various agencies including, but not limited to, education, employment, justice, and family services.
- Identify and mitigate barriers to treatment adherence (for example, organization transportation)
- Create Individual files for each client and do follow up, weekly, to ensure that program is being run efficiently.
- Ensure a good communication plan for Mental Health Clinicians to create.
- Utilize a circle of care teams to help with the process for each client.
Dispensing and Monitoring 20%
- Witness dispensing of buprenorphine(suboxone) out of the health center to ensure ongoing contact with clients, providing opportunities for routine informal check-ins for individuals who do not actively seek out programming and support.
- Conduct witnessed urine drug screening as required.
- Conduct medication counts as required.
Community Outreach 10%
- Organize and implement community and family education on the treatment of opioid use disorders.
- Organize stakeholder meetings
- Facilitate collaboration with various community agencies to foster support for individuals in recovery
Qualifications and Experience/Skills
- Bachelor’s Degree in related discipline such as psychology, counselling, social work, or addiction studies. A combination of education and experience will be considered.
- 2+ years’ experience in a clinical or medical office setting .
- Strong verbal and written communication skills are important.
- Professional and calm demeaner with the ability to respect the confidentiality of others.
- Ability to lead by example in maintaining a positive attitude and friendly demeanor.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and general office equipment.
- Familiarity with Opioid Use Disorder and Treatment.
- Harm Reduction and person-centered approaches used.
- Navigation of public services, such as withdrawal management services.
- Ability to work as a part of a team.
- Knowledge of and/or experience working in an Indigenous and/or First Nation communities/organizations is an asset.
- Must have access to reliable transportation.
- A Criminal Record and Vulnerable Sector checks are required.
Working Conditions
- 5 days per week, Monday to Friday 8:00am to 4:00pm daily, 35 hours weekly
