POSITION: Family Support Worker
SUMMARY: Under the direction of the Supervisor, Family Support, the family support worker(s) are responsible for contributing to the general unit administration; for participating in service coordination; for providing direct support to individual clients and families, and for the performance of other related duties as assigned by the Supervisor.
STATEMENT of DUTIES/RESPONSIBILITIES
- Contributes to general unit administration by:
- Participating in and contributing to unit staff meetings respecting unit problem-solving, evaluation, goal and objectives setting and work planning;
- Assisting with the establishment of operational service criteria;
- Maintaining daily activity logs
- Participates in service coordination by;
- Working cooperatively with the service coordinators in the process of situation investigation and information gathering relevant to client needs;
- Participating in the assessment of client needs (children & family members);
- Working cooperatively with the service coordinators in the provision of brief service to clients
- Participating in the development and modification of service plans;
- Participating in case reviews and conferences;
- Liaising with other service providers as appropriate;
- Continuously updating the service coordination information system as service provision takes place (eg. Case, notes, contacts, etc)
- Provides direct support to individual clients and families by;
- Maintaining open, supportive communication with assigned clients;
- Providing support, individual and/or group counseling and socio-relational, life skills, parenting and other client training as called for in the client service plan- through home visits, workshops, in-office meeting or meetings at mutually agreeable locations;
- Providing transportation where appropriate in the context of the developing client/worker relationship;
- Continuously re-assessing client needs in relation to service plan goals, objectives and activities and recommending plan changes, including referrals to other services as considered appropriate;
- Promoting, among clients, the value of meeting and sharing experiences with others who face or have faced familiar problems;
- Acting as a resource to self-help groups as requested by the Community Support staff.
- Performs other related duties as determined by the Supervisor of Family Support, which includes, but is not limited to, supporting/taking part in the Six Nations Emergency Control Plan.

- Will be thoroughly familiar with the relevant legislation, regulations and guidelines;
- Will be knowledgeable about Hadenosaunee Culture and the contemporary characteristics of and social issues within Six Nations social structure;
- Will have knowledge of traditional approaches to helping;
- Will be thoroughly familiar with Six Nations Child & Family Services principles, policy and other related service resources;
- Will have extensive knowledge of family systems and of the range of methods and techniques that are employed in Social Work practice with individuals and families;
- Will have extensive knowledge of assessment and diagnostic techniques
Ability Requirements
The successful applicant(s) will demonstrate the ability to;
- Work cooperatively with other staff and management;
- Relate effectively to community members and, in particular, to elders;
- Effectively interpret client/family needs in relation to services/resources available within and external to Six Nations’ programs; and to plan; implement, monitor and evaluate services;
- Update information systems, review information system data and prepare/present concise reports and legal documents.
Working Conditions
This position involves;
- Considerable consultation with internal staff/clients;
- Some project-specific external contact with other community service providers;
- Direct client contact with some risk due to predisposition of some clients;
- Some travel in the community as required using own transportation;
- Possible variance in work hours to accommodate clients; work is largely of a planned nature, with few interruptions other than occasional client emergency;
- Works in climate controlled environment.
WORKING RELATIONSHIPS
Works with;
- The Supervisor of Family Support/the Manager, Child & Family Services- receives direction, guidance and discusses plans, priorities, or interacts to ensure tasks are done efficiently and effectively; receives instruction and Supervision;
- Other staff/other Six Nations agencies-with courtesy, cooperation and teamwork;
- With public/clients- represents and promotes the Six Nations Child & Family Services Program in a courteous, positive, and cooperative manner, provides information and assistance;
- External agencies – represents and promotes Six Nations interests related to Child & Family Services Program; maintains awareness of legislation, policy and program changes; seeks to develop sound professional working relationships.
IMPACT OF ERROR
Errors in judgment and in the conduct of duties could compromise the safety and well-being of clients, resulting in loss of creditability, poor public relations, public confusion, and legal liability to self and to the Six Nations Council.
