Position: BSO Personal Support Worker

Reports to: Director of Care
The Behavioural Support Personal Support Worker is part of our Southbridge Care Home team that provides direct care, education and support to our residents who are demonstrating meaningful expressions that may be associated with various types of dementia, delirium,
mental health, addictions or other neurological conditions. The BSPSW will contribute by sharing expertise and assistance with personal care and assistance with activities of daily living for persons served within the program with responsive expression challenges. The BSPSW is an individual that will be responsible for coordinating with internal and external stakeholders for the development of a person-centred-holistic approach to resident care.
Key Responsibilities:
- Contribute to delivering person-centered care by promoting the abilities and self esteem of the residents, develop strategies to promote optimal quality of life for the residents with the responsive behaviors.
- Conduct assessments using various assessment tools which describes behaviors of residents including observational behaviors, identifying behavioral risks, and identify caregiver strategies.
- Ensure the plan of care is regulated with responsive behaviors of residents in regard to the staff providing care and mitigating risks by conducting weekly rounds.
- Communicate with the external stakeholder and management team to identify the risks associated with responsive behavior.
- Demonstrate participation in quality improvement activities and provide monthly reports.
- Provide coaching and mentorship to the staff to ensure collaboration of care and services for the responsive behavior.
- Conduct resident care reviews and attend quality program meetings including P.A.C.
- Collaborate with the pharmacists and physicians for the consultation regarding the effect of medication treatment on residents.
- Participate in the staff development programs and seminars and require to attend ongoing training to support skills in managing residents with responsive behaviors.
- Assist in tasks by considering policies of the home and the behavioral support Ontario project.
- Maintain amazing relationship with all the stakeholders including staff, residents and the general public.
- Provide an excellent customer service to all the stakeholders and assist in the administrative duties of the home.
- Other duties as required.
Knowledge and Skills Required:
- Completed a PSW Program that meets the appropriate standards established by the Ministry of Training, Colleges and Universities, the National Association of Career Colleges, or the Ontario Community Support Association and must be a minimum of 600 hours in duration (including class time and practical experience time)
- Knowledge and proven experience working with residents in Long-Term Care
- Knowledge of GPA, P.I.E.C.E.S., U-First or willingness to receive such training.
- First Aid and CPR certification.
- A serving heart and a passion for caring for seniors.
- Ability to interact with residents, families and staff in a positive and supportive
Manner.
- Ability to work individually and as part of a team, to prioritize tasks in order of urgency.
- Excellent communication (verbal and written) and comprehension skills.
- Promote safe work practices within the facility and ensure all legislative
requirements under the Occupational Health and Safety Act are complied with.
Additional Requirements include
- A negative vulnerable sector check completed within 6 months of start date.
- TB screening documentation as required under the Southbridge Care Homes’ Tuberculosis (TB) Screening Policy
