Position Summary
As a member of the Integrated Care & System Partnerships portfolio, the Program Manager is a leader who will support the development, delivery and ongoing management of transitional and community care programs aligned to the portfolio. Reporting to the Director, Community Integration and Outpatient Transformation, and in collaboration with internal and external partners, the Program Manager will oversee Sunnybrook programs supporting transitions of care, such as the Sunnybrook-to-Home Program and the Centralized Community Health Professions Team, as well as providing management and operations leadership to the Pine Villa Transitional Care Site.
Key Responsibilities
- Serve as an operational point of contact for transitional and community care programs within the portfolio.
- Chair program and care-related committees and working groups.
- Report and monitor deliverables across multiple programs.
- Identify, mitigate and appropriately escalate program and project risks to the Director and senior leadership.
- Promote strong collaborative relationships with internal and external stakeholders.
Sunnybrook-to-Home / Hospital-to-Home
- Provide operational leadership and management oversight of Sunnybrook-to-Home and other transitional care initiatives.
- Manage relationships and contracts with partners to ensure relevant standards, accountabilities and deliverables are consistently met.
- Work with internal programs and external partners to identify, implement and evaluate opportunities to optimize patient transitions.
- Collaborate with program leadership, Decision Support and Finance in the development and monitoring of performance measures.
- Analyze data and provide regular updates to internal and external leaders, including Ontario Health.
Centralized Community Health Professions Team (CCHPT)
- Supervise, support and manage health professional staff within CCHPT.
- Work collaboratively with participating sites to identify and address service gaps and optimize care delivery.
- Manage relationships with internal programs, site leadership and other key partners to support program delivery.
- Lead ongoing program development and identify opportunities to expand or enhance CCHPT services.
- Collaborate with Professional Practice to support standardization and optimization of professional roles.
- Monitor program performance, deliverables and Ontario Health reporting requirements.
Pine Villa Transitional Care Site
- Provide management and operational oversight of the Pine Villa Transitional Care Site.
- Supervise, support and manage Sunnybrook health professional staff at Pine Villa.
- Oversee site operations, supplies, equipment and building maintenance to support the safe and smooth functioning of the site.
- Collaborate with internal departments and external partners to address site operational needs.
- Support site safety, emergency preparedness and communication/escalation processes.
- Identify, mitigate and appropriately escalate operational and site risks.
Education And Experience
- Masters degree required (preferably in a health related discipline or health administration)
- At least five years of recent or related progressive health care and management experience
- Experience in a leadership role managing teams involved in clinical care or complex projects and demonstrated project management experience is an asset
- Proven management skills including financial, human resources, quality improvement, project planning and risk management skills
- Have a strong understanding of the health care environment including funding methodologies and hospital data structures (clinical and financial)
- Excellent interpersonal skills, including communication, conflict resolution, organization skills, critical thinking, decision making and problem solving
- Demonstrates ability to work in a team environment and collaborates with others in assisting with the delivery of care to patients
