Original job description

Organization

The Vancouver Division of Family Practice (VDoFP) is a not-for-profit organization with a mission to improve the primary care system in the City of Vancouver for the benefit of our family doctors and their patients. We are funded by the Government of British Columbia and Doctors of BC, and work in partnership with Vancouver Coastal Health, Providence Health Care, First Nations Health Authority, community health organizations and the BC Ministry of Health.

Team

VDoFP and VCH are collaboratively establishing a Ministry of Health-funded Primary Care Network (PCN) model to create an integrated system of care. In the Vancouver PCNs, family physicians and nurse practitioners (via Patient Medical Homes); registered nurses, allied health professionals, and other health care providers all work together to provide effective and comprehensive, patient-centered primary care services. The VDoFP PCN team works closely with the VCH PCN team, to build primary care networks in Vancouver that include the voices of all stakeholders.

Position

To deliver on the above, we are looking for the right person to join our team of Community Network Managers (CNMs). Our CNMs are the primary facilitators for the PCNs, with a strong focus on physician engagement, proactive outreach, relationship building, and delivering exceptional service to network members.

CNMs serve as a key point of contact for family physicians and clinic staff, developing trusted relationships, understanding their needs, facilitating connections, and helping them access available services and resources that support clinic operations and overall network effectiveness.

The work is very fluid and requires an adaptable, creative approach. Successful outcomes require the CNM to work independently and collaboratively with various teams across Vancouver.

Duties & Responsibilities

As a Community Network Manager your focus is the following:

1. Establish and maintain strong relationships with family physicians, clinic staff, and other network partners, encouraging active engagement and participation in the PCN

  • Proactively connect with clinic owners, physicians, Medical Office Assistants (MOAs), and other clinic staff to understand their individual and collective needs and how the PCN can offer support.
  • Build and maintain trusted, long-term relationships through regular outreach, responsive communication, and personalized service.
  • Drive effective engagement and communication between network members through tailored events, communications to different network member groups, and by building communities of interest/practice.
  • Act as a key point of contact for physicians and clinic staff, responding to inquiries, identifying needs, and connecting them with appropriate resources and supports.
  • Gather and maintain important physician and clinic data that helps the VDoFP better understand the clinics and people in the networks and evaluate the effectiveness of engagement efforts.

2. Support clinics by understanding their needs and connecting them with appropriate resources and services

  • Become the “trusted advisor” to clinics in the PCN(s) and build a thorough understanding of their operational challenges
  • Identify opportunities to support clinics and connect physicians and clinic staff with appropriate resources, services, and subject matter experts.
  • Promote awareness of available PCN programs, services, and initiatives that can support clinic operations and patient care.
  • Encourage and support physicians and clinic staff in participating in opportunities for quality improvement, collaboration, and practice optimization.

3. Support physician engagement and communication related to PCN services and resources

  • Develop an understanding of the network's Service Plan and the services and resources available to participating clinics.
  • Build awareness and understanding of PCN services and resources through proactive physician outreach and ongoing communication.
  • Support clinic readiness and the introduction of new resources by facilitating communication, gathering feedback, and connecting clinics with appropriate team members.
  • Maintain ongoing relationships with clinics to identify challenges, address concerns, and support the successful integration of PCN resources.
  • Gather relevant feedback and engagement data to support continuous improvement and inform PCN planning.

4. Other duties as assigned

Qualifications and skills:

A successful Community Network Manager brings the following to the role:

  • Three to five years of relevant experience, preferably in a client-facing role involving relationship management, stakeholder engagement, community outreach, or customer service.
  • A post-secondary degree, for example, in healthcare, public health, public administration, business administration, or a related field.
  • Demonstrated ability to build rapport, establish trust, and maintain positive relationships with diverse individuals and organizations.
  • A proactive, service-oriented approach, with strong interpersonal skills and a genuine enjoyment of connecting with people.
  • Likes people and has a partnership attitude - easily builds and maintains relationships with the people and organizations in the network.
  • Desire to make a difference - sees the way things could be and works to make it happen.
  • A change agent - likes variety and is comfortable encouraging people to embrace change to make things better.
  • Organized - capable of managing multiple priorities, maintaining regular follow-up, and balancing competing demands.
  • Excellent verbal and written communication skills to engage with physicians and clinic staff, host events, and prepare communications, presentations, and reports.
  • Strong computer skills, particularly Microsoft Office and Google Workspace.
  • Comfortable with data collection, documentation, and maintaining accurate records, with a solid working knowledge of spreadsheets.
  • Willing to travel across the City of Vancouver to connect with clinics (physicians and staff) and other network partners.
  • Bonus points for familiarity with the primary care system and available community healthcare services in Vancouver.

This is a 12 month contract position with the possibility of extension or moving to permanent employment. The expected salary range is $72,000 - $75,000 per year.

We aim to foster a division of family practice that celebrates and reflects the diversity of the communities we serve. It takes diversity of thought, culture, background, and perspective to create meaningful change in primary care. The Vancouver Division is committed to promoting equity, diversity and inclusion in our recruitment process where all applicants are considered regardless of race, colour, ancestry, place of origin, religion, family status, disability, sex, age, sexual orientation, gender identity and other protected grounds.
