Yee Hong Centre for Geriatric Care (Yee Hong) is a recognized leader in senior care, dedicated to providing exceptional, culturally inclusive services that enhance the well-being of older adults. With a strong reputation for excellence in long-term care, community programs, and aging-in-place initiatives, Yee Hong is committed to supporting seniors in living with dignity, independence, and the best possible quality of life. At the core of its mission is a focus on compassionate, person-centered care tailored to the unique needs of diverse communities.

Yee Hong is one of the foremost providers of culturally appropriate senior care in the country, delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors’ apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.

Program Coordinator, Home Support Services (Part-Time)

The incumbent is responsible to the Program Manager/Assistant Director of Home Support Services, for the overall day to day coordination of Support Services to the community

Responsibilities:

Coordinates the delivery of appropriate home support services to enable clients to age in place
Coordinates meal and transportation services for clients
Provides timely and professional client intervention to clients and family
Monitors overall health status and keeps up-to-date health records of clients
Coaches, develops and evaluates staff to ensure they meet their required performance standard. Accurately schedules staff resources in accordance with clients' needs as reflected in the care plan
Ensures implementation of risk management policy and safe environment for clients and staff
In consultation with the Manager, develops and updates policies, procedures for the program
Coordinates the optimal deployment of volunteers for the program and provides effective guidance, training and support to the volunteers
Maintains up to date and accurate files, records and statistics on the program
Ensures efficient and responsible use of resources and maintains accurate record of fees and expenditures
Provides emergency response to emergency calls from staff and clients
Effectively promotes the home support services to clients of the building/neighbourhood
Keeps up-to-date with developments and best practices in the field
Other duties as assigned

Qualifications:

Bachelor's degree in social services, health care, activation, or equivalent
Minimum 2 years experience in social services or health care, supervisory experience an asset
Knowledge and experience in working with seniors
Computer proficiency, with knowledge of assessment software, AlayaCare, and Microsoft Office Solutions
Current CPR Certificate required
Satisfactory Police Reference Check (with vulnerable sector screen) result
Language Requirement: Ability to communicate in English and Chinese

Physical Demands/ Working Condition:

Occasional travel
Use of own vehicle
