As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.

Required Qualifications:
Completion of an accredited practical nurse education program. Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
5+ years’ experience in good standing with CLHA; Experience with Patient and Family Center Care (PFCC) and Complex Discharge Management with a multi-disciplinary team; Skill in collaborative practices and concurrent care planning; Case Management experience (Homecare or CCH environment); Able to navigate difficult conversations with a focus on solutions; Demonstrated ability to provide comprehensive assessments to individuals within their physical and social environments; Able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines; Able to tolerate long periods of sitting or standing as well as repetitive movements; Able to lift and carry 10-15lbs regularly; Able to respond with hands-on care of patients during assessment (transfers, bariatric clients, bending, twisting, walking, intervening for client safety as required); Able to provide physical assistance to clients and intervene to ensure client safety.
Preferred Qualifications:
Minimum of two years' experience within Home and Community Care; Completion of an LPN program from an approved college; Trained in RAI HC and interRAI Contact Assessment; Experience in a CCH (Type A, B, or C) environment; Specialty training in geriatrics or palliative and end of life care.
