C-Care Health Services is a Toronto-based home healthcare agency providing professional and compassionate care services to clients and families across the Greater Toronto Area. We are seeking a Client Care Coordinator who shares the same values in providing the best quality of care to join our team.
Position Summary
The Client Care Coordinator is responsible for providing providing support to the clients and staff through proper communication and care coordination. The role is the main point of contact for concerns relating to client health and condition.
Key Responsibilities
- Conduct initial and ongoing in-home client assessments as needed for communicating services provided by the agency, updating of care plans, training of staff, etc.
- Create and update care plans for new and existing clients, and ensure that it is accurate and in accordance to the client's condition and needs
- Discuss the care plan to the staff and ensure that any changes or updates are being relayed to the appropriate staff in a timely manner
- Respond to client or family member inquiries relating to the client's care and condition
- Communicate with the client or family members any concerns, significant events, or scheduling changes that might impact the care being provided
- Monitor staff performance including but not limited to attendance, job performance, conduct, reporting, etc.
- Monitor shift notes and incident reports received from the staff and take appropriate action as needed
Qualifications
- Current and active CNO RN or RPN license
- Valid BLS certificate
- Good verbal and written communication skills
- Previous experience in care coordination is an asset
- Previous client management experience is an asset
