About the Role
The Independence Coach brings occupational therapy knowledge and clinical reasoning into CONNECT’s Life Redesign approach.
You will work with each person to understand what matters to them, what supports or limits their participation, and what practical strategies can help them build a meaningful life.
The people supported at Cambie Gardens may have complex physical, cognitive, neurological and health needs. Many use wheelchairs, making seating, positioning, mobility and pressure management an important part of the role.
What You'll Do
- Learn about each person's strengths, interests, relationships, goals and priorities.
- Assess participation in meaningful activities, including self-care, home life, relationships, leisure and community activities.
- Develop practical and measurable strategies with the person and their support team.
- Adapt activities, environments, equipment and routines to promote independence, safety and participation.
- Assess and support wheelchair seating, positioning and mobility.
- Provide recommendations related to pressure management and pressure injury prevention.
- Develop practical strategies to support memory, attention, planning, problem-solving and other cognitive needs.
- Work directly alongside Life Redesign Coaches in people's homes and communities.
- Coach and support staff in applying OT strategies during everyday routines.
- Collaborate with families, Health and Wellness Coaches and other professionals.
- Contribute OT recommendations to one integrated Life Redesign Plan.
- Maintain clear and timely clinical documentation.
- Participate in team learning, reflective practice and quality improvement.
- Respond to changing needs and participate in the urgent/emergency support network as required.
What We're Looking For
Required:
- Occupational Therapist licensed and in good standing with the College of Health and Care Professionals of BC.
- Experience supporting people with complex physical, cognitive and/or neurological disabilities.
- Experience working in home or community-based settings.
- Experience translating cognitive assessment into practical, everyday strategies.
- Strong clinical judgment and professional accountability.
- Excellent communication and collaboration skills.
- A strengths-based, relationship-centred approach.
- Ability to work creatively in people's homes and community settings.
Strongly preferred:
- Experience with wheelchair seating, positioning and mobility.
- Knowledge of pressure management and pressure injury prevention.
