Qualifications:

Education and Training-

Must be 18 years of age or older
University Degree or DSW or college diploma in a related Human Services field, or certification in a related field (ex. PSW certificate) with a minimum of 2000 hours of experience at Manager’s discretion
Grade 12 or GED with a minimum 4000 hours experience at Manager’s discretion
Valid driver’s license and required insurance
Able to obtain required training; including but not limited to; CPR, First Aid, NVCI, WHMIS, Violence in the Workplace, fire extinguisher, Bullying in the Workplace, Hand Hygiene, Donning and Doffing and use of Personal Protective Equipment (PPE)
Attend Association orientation

Responsibilities and Duties:

Programs- Follow programs as outlined in the Individual Support Plan (ISP) and assist people to achieve their goals.

Offer input into the creation of the ISP, honouring personal choice and informed decision making
Support and encourage each person, where appropriate, to develop skills and assume roles, which promote their participation in the community
Ensure all activities are in accordance with the principals and philosophy of Elmira District Community Living
Support people, where appropriate, in decision making skills, exercising choices, taking reasonable risks and assuming responsibilities. Demonstrate good judgment when decisions may place the individual at undue risk
Provide respectful and supportive personal care where needed
Identify organizational barriers to people’s goals and promote alternative strategies and/or approaches

Support and encourage each person to participate in the cultural, spiritual, recreational, leisure and residential activities in the community
Advocates on behalf of people supported, for the agency and for the sector
Advocate on behalf of the person when necessary and in conjunction with family/primary caregiver, to outside agencies for consultation. Ex. appointments such as medical, OT/PT, behavioural, etc. recreational and financial or any other areas as needed

Responsibilities to Family/Guardian/Advocate-

Act as liaison
Make Families/Guardians/Advocates welcome in the home/work place

Administrative Duties-

Maintain complete, up-to-date and confidential records and documentation for each person
Submit monthly availability form to scheduling in accordance with deadline dates

Supervisory-

Supervises daily living skills to promote independence where deemed appropriate including; meal preparation, clean up, laundry and general household duties. Supervises and carries out care of personal hygiene
Supervises recreation and leisure activities within the home and community as required

Report to:

Report any problems/concerns to Manager or Supervisor immediately

General Duties for Overnight Shifts-

Completes nightly attendance check
Ensures that everyone’s needs are met and that no safety hazards exist in the building before retiring for the night
Completes necessary night checks before retiring for the night
Ensures that any required household chores are complete before retiring for the night
May occasionally be required to be awake during the overnight to attend to illness or other situations requiring staff supervision – fill out required paperwork associated with this
Prepares and/or serves breakfast assisting with menu preparation as necessary
Supervises the morning routine by ensuring that people are awake in time to prepare for the day activities, and personal care and hygiene routines are followed

In conjunction with the person; assure that proper attire is worn for program or activity, assures that a nutritious breakfast is prepared and eaten, supervises clean-up of breakfast dishes and dining area
Ensures that people are ready for transportation as required

Team Function-

Attend work punctually and maintain acceptable attendance standards

Act as liaison between programs, staff and the community sharing pertinent information with Manager/Supervisor and relevant team members
Attend, participate and contribute to staff meetings in a positive and respectful manner
Attend and participate in meetings, discussions and decision making as Manager deems necessary
Communicate any information which may be included in the Individual Support Plan, to Primary Support Staff.
Conduct themselves in a positive, respectful and professional manner at all times
Attend and contribute to all staff development
Maintain confidentiality between programs in relation to staff issues and performance
Provide ongoing information to all those involved with people we support, including Manager, Supervisors, staff and homes/programs
Fosters, on a day-to-day basis, the inclusion of all co-workers and promotes team work and cooperation
Maintain accurate communication notes where applicable
Communicate positively at all times with regard to staff and people we support; acts as a role model
Respect everyone’s confidentiality, including co-workers and this organization
Work within agency policy to resolve issues and concerns within the program to reach a constructive and positive outcome

Health and Safety-

Encourage people to assume as much responsibility for their health care needs as possible through instruction and education
Aware of all hand hygiene, donning and doffing and correct use of all Personal Protective Equipment (PPE)

Ensure that Health and Safety Policies, guidelines, incident/accident reports and occurrences, NVCI or other crisis protocols are understood, followed and reported
Will possess and demonstrate knowledge and certification (where applicable) of First Aid, CPR, NVCI, WHMIS, Occupational Health and Safety, fire prevention and other areas as deemed necessary by this Association
When required, must be able to demonstrate safe lifting techniques for both mechanical and manual lifts and according to EDCL policy.
Complete monthly fire drills; be aware of evacuation and emergency procedures
Ensure that all medications are administered and documented accurately and are kept in the designated, properly locked cabinet
If driving Association vehicle, ensure that circle check is completed as well as Ministry checklist and vehicle meets the standards of the MTO (if applicable); ensure van cleanliness

Working Conditions-

May be required to accommodate a person’s changing physical or environmental needs, ex. aging, dementia, mobility or behavioural
Adapting to uncontrollable circumstances, for example, power outages, severe weather
Ensure that the home/work environment is a clean, safe and enjoyable environment and meets or exceeds the standards of the Ministry of Children, Community and Social Service
Ensure that the home or support environment supports people’s emotional and physical well being
Provide assistance with other daily living/work duties as required
Transporting people as needed in agency vehicles or in personal vehicle as approved by

Manager/Association

May be occasionally required to perform basic maintenance duties
