Pill4Me is a community pharmacy in Chilliwack focused on improving access to medications and healthcare services. We work closely with patients, prescribers and community organizations, including supporting individuals receiving opioid agonist treatment (OAT) and other substance-use care.
The Role
We are looking for a Licensed Practical Nurse (LPN) to join our community outreach team.
This is a field-based role focused on medication delivery and supporting patients who receive OAT and other prescribed therapies in the community. The LPN will work closely with the pharmacist while independently completing scheduled community visits throughout Chilliwack.
Key Responsibilities
- Deliver medications to patients in the community
- Witness medication administration or ingestion where permitted within professional scope and applicable requirements
- Support patients receiving OAT and other prescribed therapies
- Follow pharmacy procedures for patient identification, medication delivery, security and chain of custody
- Accurately document deliveries, witnessed doses, missed doses and relevant patient interactions
- Promptly communicate missed doses, clinical concerns or changes in patient condition to the pharmacist
- Coordinate with pharmacists, prescribers and community healthcare partners as required
- Maintain respectful, professional and non-judgmental relationships with patients
Qualifications
- Current registration as a Licensed Practical Nurse (LPN) in good standing in British Columbia
- Comfortable working independently in community settings
- Strong communication, documentation and time-management skills
- Professional and non-judgmental approach to substance-use care
- Experience with OAT, substance-use care, mental health, harm reduction or community nursing is an asset
- Familiarity with electronic documentation and mobile technology is an asset
