Senior Social Worker - Transitions Team
Pro Social Partners are working in partnership with Medway Council to recruit a Senior Social Worker into their Transitions Team.
This is a full time permanent role supporting young people as they move from children's services into adulthood, a rewarding area of practice that bridges children's and adults social care.
The role
- Holding a caseload of young people aged 14 to 25 preparing for adulthood
- Carrying out assessments under the Care Act 2014 and the Children Act 1989
- Planning transitions around education, health, housing and independence
- Working across children's and adults teams and with health partners
About You
- Registered with Social Work England
- A recognised social work qualification
- Experience with young people with disabilities or complex needs
- Confident working across children's and adults legislation
The package
- Salary of £47,435 per year
- A permanent senior role in a supportive Kent authority
