Rainbow Valley Community Health Centre (RVCHC) is in the Ottawa Valley in the village of Killaloe, Ontario. RVCHC is the only Community Health Centre in Ontario to be administered by a hospital, St. Francis Memorial Hospital (SFMH). The team is comprised of part time staff including Physicians, Nurse Practitioners, Registered Nurses, administrative staff and Social Worker.

Pay Grid: $47.74 - $51.70 /hour
The Rainbow Valley Community Health Centre Social Worker functions as a member of the primary health team in the provision of assessment, individual and family counseling, case management and facilitation of groups operating within the values and objectives of the Centre. Crisis intervention services, in partnership with the other mental health programs in the County, are also an element of the position. The Social Worker works closely within the inter-disciplinary team, providing consultation to members of the team.
RESPONSIBILITIES:
- Supports the functioning and independence of individuals and families by providing counseling services to individuals and families.
- Provides information and advice to clients of the Centre and residents of the catchment area who are seeking support for mental health issues, and/or issues impacting on their ability to cope.
- Participates in case conferences, and provide case management to clients and families as required.
- Provides mental health consultations and information to staff of the Centre.
- Develops and implements mental health support groups, mental health promotion groups and groups that support clients with mental health issues.
- Ensures appropriate interventions are delivered by maintaining proper client records, assessment notes and correspondence.
- Ensures appropriate encountering of services in the Centre’s electronic charting system.
- Contributes to the development of new programs that address community health needs by collaborating with Centre staff and other agencies to identify, recommend and/or develop resources and materials and by participating in the delivery of programs as required.
- Provides leadership for appropriate community educational programs in collaboration with Centre staff and partner agencies.
- Attends and participates in regular staff and team meetings, and staff & board events.
- Participates in the selection and supervision of students and professionals seeking placements pertaining to counseling at the Centre.
- Identifies appropriate clients who could benefit from participation in the LHIN’s Health Links approach. Works with clients to develop Coordinated Care Plan to meet RVCHC annual target.
- Participates in the development of relevant policies, procedures and best practices for the Centre.
- Initiates and maintains partnerships with relevant community agencies and professionals.
- Promotes community awareness of and participation in the Centre’s programs.
- Provides support and supervision to students who have placements at the CHC.
- Plans and participates in outreach activities related to primary health care.
- Works in a manner that preserves confidentiality, seeks to minimize risk, incorporates health promotion and recognizes the determinants of health.
- Respects and values the diversity of communities and individuals.
- Maintains competence, and where applicable, a professional license to practice.
- Contributes to and participates in the Continuous Quality Improvement Program by assisting to implement changes as required to improve services and enhance departmental efficiency.
- Accurately and consistently adheres to all Clinic and relevant Hospital policies and procedures.
- Complies with SFMH Ethics Statement and the Joint SFMH/VM Ethics Advisory Committee – Submissions Policy & Procedure.
- As defined in the hospital Occupational Health and Safety Roles and Responsibilities, performs all work in a safe manner including infection control, client handling program, WHMIS, all emergency codes etc.
- Upholds the ethical and legal responsibility to maintain the confidentiality and privacy of client health information obtained while providing care, in accordance with the Personal Health Information Protection Act (PHIPA).
- Establishes and maintains respectful, collaborative and professional relationships that include relationships with colleagues, health care team members and employers. Professional relationships are based on trust, respect and by upholding the code of conduct as set out by St. Francis Memorial.
- Assumes other related duties as required.
QUALIFICATIONS:
- A Registered Social Worker with a minimum of a Bachelor’s Degree and two to four years of counseling experience (or an equivalent combination of education and experience). Master’s degree preferred.
- Extensive assessment and counseling skills;
- Thorough knowledge of and proficiency in current assessment and psychosocial counseling techniques;
- Strong interpersonal and communication skills;
- Ability to work effectively within an inter-disciplinary team;
- Commitment to and knowledge of community-based health care, preferably in a rural setting;
- Sensitivity to people with a variety of health, social, cultural and economic needs.
