REGENT PARK COMMUNITY HEALTH CENTRE
We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services. Community Health Worker, HART Hub
We are currently hiring for two (2) Full-Time Permanent positions, 35 hours per week
Hourly Range: $32.10 to $38.67, plus benefits and HOOPP Pension
Position Summary
As a member of the Homelessness and Addiction Recovery Treatment (HART) Hub Team, the Community Health Worker supports individuals in the downtown east community of Toronto who are experiencing homelessness, serious mental health and/or substance use issues to connect them with necessary services, such as housing, healthcare, addiction treatment, and mental health services. The Community Health Worker (CHW) provides intensive, low barrier individualized case management supports to individuals ensuring a cohesive and client-centered approach to addressing complex service needs such as homelessness, substance use, mental health challenges, and unemployment.
Through intensive case management, the CHW will work to address social determinants of health for clients looking to access community-based treatment and recovery options and will utilize health promotion, community engagement, social justice, trauma-informed, and anti-racist/anti-oppressive (ARAO) frameworks to promote the mission and values of RPCHC.
Responsibilities Include:
- Conduct comprehensive health and social needs assessments to identify barriers to care, determine eligibility for services, and support the development individualized care plans.
- Identify and refer clients to appropriate treatment options for substance use and mental health disorders, as well as housing programs, health care services, legal assistance, and social services.
- Support the coordination of services, including addiction treatment, mental health care, medical care, and housing support, to create an integrated support system for the individual.
- Establishing trust and rapport with the individual to create a safe and supportive environment.
- Respond to emergency situations, including mental health crises, overdose situations, or moments of acute distress, and ensuring the individual receives immediate intervention or stabilization.
- Use de-escalation techniques to manage situations where an individual may be experiencing severe emotional distress, aggression, or confusion.
- Ongoing emotional support to help the individual stay engaged in treatment and recovery
- Provide education on health topics such as managing chronic health conditions, complex disorders, hygiene, and overall wellness.
- Work collaboratively with the community, community partners and stakeholders to develop and implement action plans that address issues around health, poverty and homelessness.
- Support with the coordination, facilitation, and delivery of group programming.
- Advocate for individual client needs to ensure access to effective care, follow-up and resources from both an individual and systemic level
- Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs
- Participate in relevant meetings and actions as per internal policies and protocols;
- Engage with HART Hub advisory groups representing the needs and experiences of people who use HART Hub in order to adapt and improve program delivery and outcomes
- Participate in the development and/or support of policies and strategies which are in the interest of public health recognizing the social determinants of health
Required Knowledge, Skills, and Abilities:
- Post-secondary education in social work, social service work, community work, health promotion, adult education and/or equivalent in life experience.
- A minimum of 3 - 5 years of community based experienced working with marginalized and homeless populations who experience multiple systemic barriers.
- A minimum of 3-5 years proven case management and system navigation experience with people experiencing homelessness and systemic barriers
- Proven experience engaging in street outreach and low threshold models of engagement and support
- Knowledge of and demonstrated ability to support the homeless population and apply trauma informed principles in work with clients and community.
- Knowledge of overdose prevention and response strategies
- Excellent understanding of impacts of systemic oppression on marginalized individuals and communities
- Demonstrated understanding of principles of community development and experience applying these principles in work settings.
- Extensive experience in conflict resolution and crisis intervention
- Extensive knowledge of mental health, addictions and crisis services and systems
- Awareness of and sensitivity to the health issues of a diverse community and experience working with disenfranchised populations.
- Experience and demonstrated ability in community development, advocacy and peer support.
- Demonstrated interpersonal, decision making, conflict management, critical thinking, organizational skills and ability to work collaboratively within and across teams
- Strong oral and written communication skills
- Must be drug and sex positive.
- Proficiency in the use of computer technology and various software applications.
- Understanding of Anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
- Knowledge of the Regent Park area and issues affecting the community
- Commitment to valuing diversity, equity, partnerships and advocacy in keeping with Regent Park Community Health Centre’s Strategic Direction.
- Must be fully vaccinated for COVID 19 for the protection of clients and staff
