The PCN will build and maintain relationships with outside agencies, programs, and partner organizations to enhance client care and facilitate service navigation. The PCN will be a welcoming and compassionate person and will possess knowledge of social issues such poverty, food insecurity, substance use, mental health/illness, and racism.
