Key Responsibilities:
- Bachelor's Degree or Community College degree in a health-related discipline
- Registration with a recognized college
- Minimum of 3 years of postgraduate experience in Mental Health; preferably in the community and hospital setting
- Case Management experience in supporting people living with Psychosis Spectrum Disorders
- Excellent interpersonal, communication and organizational skills
- Positive work and attendance record
- Ability to work collaboratively with patients, families and other healthcare professionals
- Exceptional problem-solving and decision-making skills
- Demonstrated ability to work independently and as a member of an interdisciplinary team
- Valid driver’s license, relevant insurance and use of own vehicle is required
Qualifications:
- Member of an inter-professional team with the THP Psychosis Outpatient Program
- Support clients to stabilize their mental health, achieve their recovery goals and improve their quality of life.
- Assist clients in connecting with community resources in areas including school, vocation, housing, finances and recreation.
- Consult with community agencies and families to maintain coordination in the treatment process
- Support clients with life skills and coping strategies
- Health promotion and teaching with a focus on recovery and wellness
