Role Description The Nurse Practitioner is a Full-Time Permanent, on-site role based in St. Jacobs, ON, providing primary health care services within an interdisciplinary team environment. Day-to-day responsibilities include conducting comprehensive health assessments, diagnosing and managing common acute and chronic conditions, and prescribing medications and treatments in accordance with professional standards and regulations. The role involves collaborating with other health care providers to develop and implement care plans, coordinating referrals, and supporting continuity of care. The Nurse Practitioner also engages in health promotion and disease prevention activities, such as client education, outreach, and community-based programs that reflect a holistic approach to wellness. Additional duties include maintaining accurate clinical documentation, participating in quality improvement initiatives, and contributing to a respectful, inclusive, and culturally safe care environment.
Qualifications
- Current registration in good standing as a Nurse Practitioner with the appropriate provincial regulatory body (e.g., CNO) and eligibility to practice in Ontario.
- Demonstrated clinical skills in primary care, including comprehensive assessment, diagnosis, treatment, and follow-up for clients across the lifespan.
- Experience working within an interdisciplinary team, with strong collaboration, coordination, and care planning abilities.
- Commitment to holistic, client-centered care that considers physical, mental, spiritual, and social determinants of health.
- Strong communication skills, including active listening, clear documentation, and the ability to provide health education to diverse populations.
- Proven ability to work respectfully with individuals and communities from varied cultural, socio-economic, and linguistic backgrounds.
- Experience in community health, public health, or primary care settings, preferably in rural or small community environments.
- Skills in health promotion, disease prevention, and outreach program development and delivery.
- Ability to manage time effectively, prioritize caseloads, and adapt to changing client and community needs.
- Competence with electronic medical records and basic computer applications.
- Demonstrated commitment to ethical practice, accountability, and continuous quality improvement.
