Fraser Health invites you to join an innovative and unique primary health care team located in Langley – a fast-growing urban centre surrounded by parkland, in the heart of the Lower Mainland.
Our team provides urgent care after hours and on weekends, with a complement of 2.6 FTEs primary care providers (family physicians and/or nurse practitioners). Duties will initially focus on urgent care, with longitudinal care added in the future.
We operate in a team-based care model that includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support. You will participate as a full and active member in the delivery of team based primary care services. Together, we are dedicated to extending intentional and coordinated care to all members of the community, including the vulnerable and medically complex.
