Join the Yúustway Health and Wellness Department as a Health Care Assistant, providing compassionate, culturally safe, and relationship-centred care to Sḵwx̱wú7mesh community members in their homes. In this hands-on role, you will support clients with their daily care needs while helping promote their comfort, safety, well-being, and independence.
Working as part of the Home and Community Care team, you will provide individualized care ranging from personal care and household support to assistance for clients living with chronic health conditions. You will work closely with clients, families, nurses, occupational therapists, and other members of the care team to help ensure clients receive safe, respectful, and responsive support in their homes.
What You’ll Do
Implements approved Personal Care Plans and provides individualized support based on client needs and direction from the Manager, Home and Community Care.
Assists clients with personal care activities, including bathing, grooming, dressing, mobility, ambulation, and exercise.
Operates mechanical lifts and other mobility supports safely when assisting clients with transfers and movement.
Supports clients living with a range of health conditions, including chronic illness, and provides palliative care as required.
Maintains a clean, healthy, and safe home environment by completing household tasks identified in each client’s care plan, including cleaning, sanitizing, and laundry.
Coordinates medication administration when delegated by a nurse and supports clients with meals and nutrition.
Monitors client well-being and home environments, identifies potential health or safety concerns, and communicates observations or changes to the appropriate members of the care team.
Collaborates with nurses, occupational therapists, Squamish Nation staff, and other care providers to support client health and continuity of care.
Maintains accurate client records while respecting confidentiality, privacy, dignity, and professional boundaries.
Provides health advocacy and connects clients with appropriate health resources, referrals, programs, and services.
Participates in community development events, healing events, workshops, health functions, and other activities that support community health and wellness.
Follows workplace health and safety policies and contributes collaboratively to the Health and Wellness team.
What We’re Looking For
Certificate in Long Term Care Aide, Home Support, Residential Home Care Attendant, or a related/relevant field from a recognized post-secondary institution. A combination of education and relevant work experience may be considered.
1 to 3 years of directly related experience, including direct home support program delivery, supporting chronic illness, health promotion, community health development, and an understanding of common disease processes and conditions across the lifespan.
Compassionate, culturally safe, and relationship-centred approach to supporting clients and their families, with respect for each client’s individual needs, rights, diversity, and concerns.
Strong interpersonal and communication skills, with a professional and adaptable communication style and the ability to work collaboratively as part of a team.
High level of emotional intelligence, patience, integrity, and the ability to maintain composure in high-pressure situations.
Strong observation, organization, and time-management skills, with the ability to provide safe, individualized, client-centred care.
Standard First Aid, Level C CPR, WHMIS, and Food Safe Level 1 certifications, or willingness to obtain them within a set timeframe.
Valid BC Class 4 or 5 Driver’s Licence (Class 7N may be considered), with access to a reliable vehicle and current auto insurance that meets company policy.
Ability to work evenings and weekends as required and willingness to travel, including occasional travel to the North Shore for training, meetings, or client care coverage.
Knowledge and understanding of Skwxwú7mesh Úxwumixw history, culture, customs, land, and People is preferred.

Wage Rate: $27.88 - $39.03 hourly
