Responsibilities
Coordinate daily care planning, communication, repatriation, and discharge transitions for trauma and neurosurgery patients across the emergency department and hospital care areas. Address barriers to care, support patients and families, collaborate with hospital teams to improve patient flow, and contribute to quality improvement initiatives.
Requirements
Requires an RN with at least three years of frontline trauma and neurosurgery experience; leadership experience is required, with TL/PCC experience or DNLP preferred. Candidates need strong discharge planning and care coordination knowledge, patient-centred advocacy, effective communication and conflict resolution skills, and the ability to manage changing priorities independently and collaboratively.
