Union: SEIU Number of Vacancies: 1 New or Replacement Position: Replacement Site: West Park Healthcare Centre Department: Geriatric Functional Enhancement Service (2D) Reports to: Manager, Program Services Wage Range: $38.42 - $39.02 per hour (start rate in accordance with collective agreement requirements) Hours: 37.5 hours per week Shifts: Days/Evenings Status: Permanent Full-Time As a member of a multi-disciplinary Team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice. Use current theoretical and research knowledge and the input of team members to plan and implement care for patients and families Participate in inter-professional team care planning forums, inter-professional rounds and family conferences Provide mentorship to RPN students and new staff on the unit Participate in professional, program and hospital-wide initiatives, including professional development of self Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patientsValid registration with the College of Nurses of Ontario Diploma in Practical Nursing Recent related clinical experience in neuroscience nursing preferred Neuroscience Nursing Certification an asset Experience working with Geriatric and Frail Seniors population Effective problem recognition, problem solving and decision making skills Demonstrated commitment to continuing professional development Demonstrated commitment to team success Resilience and effective coping strategies to manage change Demonstrated computer literacy in word processing and Electronic Patient Record/HIS systems Working knowledge of the College of Nurses of Ontario standards of practice Evidence of satisfactory attendance and punctualityWhy join UHN? Responsibilities

Requirements
Requires current registration with the College of Nurses of Ontario and a Diploma in Practical Nursing; experience with geriatric and frail seniors is required, while recent neuroscience nursing experience is preferred and neuroscience certification is an asset. Candidates should demonstrate effective clinical judgment, teamwork, resilience, ongoing professional development, computer literacy, knowledge of nursing standards, and satisfactory attendance.
