Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
The Opportunity:
We are currently looking to fill 1 full-time, contract (maternity leave) Rehabilitation Services Coordinator position in our Northeast region, based in Sudbury, ON.
Wage: $31.12 per hour
Hours: 35 hours per week
Schedule: Monday to Friday, 9:00 a.m. to 4:30 p.m., with one evening shift per week from 11:00 a.m. to 6:30 p.m.
Responsibilities:
- Manage and approve program resources, including human and financial resources, to meet program goals and objectives.
- Recruit, hire, train, supervise, and evaluate support services staff within approved budgets and program plans.
- Coordinate the assessment, development, and implementation of Individual Rehabilitation Plans with assigned staff.
- Monitor and evaluate operational effectiveness and implement improvements as required.
- Participate as a member of the CSS administration team in planning, problem-solving, quality improvement, and reviewing service delivery performance.
- Provide guidance to staff and families on long-term support and service coordination for complex or catastrophic cases, helping adults with ABI remain in the community.
- Participate in community committees, where appropriate.
- Support the activities of the Community Support Supervisor by acting as a resource to staff.
- Market services for both publicly and privately funded programs.
- Fulfill on-call responsibilities.
Qualifications:
- 2–5 years of experience in the social services or health sector, coordinating services for individuals with acquired brain injuries (ABI).
- Concrete knowledge of the effects of an ABI.
- Community-based experience working with individuals with ABI, physical disabilities, or special needs.
- Demonstrated experience supervising or leading teams, including training and evaluating employees.
- Working knowledge of third-party funders and applicable legislation.
- Strong organizational, time management, and problem-solving skills, including the ability to organize, prioritize, and schedule work assignments.
- Ability to assess individual needs and determine effective compensatory strategies for individuals with brain injuries.
- Experience in budget preparation and fiscal management.
- Knowledge of human relations and behaviour management principles.
- Knowledge of health and safety legislation.
- General understanding of computer hardware and software.
- Ability to speak, read, and write in English.
- French-English bilingualism is considered an asset.
