Job Description
Schedule: Monday to Friday – 35 hours per week
Location: 23 Shingwauk Street, Garden River First Nation, ON
We are currently looking for a Program Assistant – Home and Community Care Support Services who will be responsible for providing a complete range of operational and administrative support for the assigned team in their daily activities to ensure that clients receive prompt and effective customer service. This role fosters meaningful relationships, supports client engagement, and strengthens holistic service delivery.
What You Will Do
- Provide day-to-day administrative support to the Home and Community Care Support Services Program.
- Provide support with general office duties and administrative tasks required in the health care setting.
- Receive and direct incoming telephone calls, emails, and inquiries from clients, families, caregivers, and outside community partners.
- Schedule client visits according to approved care plans, authorized service levels, staff availability, and program priorities.
- Receive and track new referrals from clients, families, physicians, hospitals, and community organizations.
- Assist with administrative processing and coordination of approved services, medical supplies, equipment, and related resources.
- Maintain referral, service, scheduling, equipment, and communication documentation.
- Assist with NIHB, Jordan's Principle, Ontario Health at Home, HNSA, MOLTC, or other funding documentation when applicable to the client's services.
- Process and assist in managing confidential patient records, ensuring compliance with privacy policies and regulations.
- Maintain Life Alert System database.
- Maintain appropriate supply and equipment records for the Home and Community Care Support Services Program.
- Assist with maintaining program statistics and administrative data.
Required Skills
- A diploma in Medical/Office Administration, Business Administration, or a related field is preferred.
- Familiarity with Electronic Medical Records and scheduling; specifically, PS Suites and AlayaCare.
- Minimum of 2 years of proven administrative experience.
- Knowledge of administrative operations, including advanced knowledge of healthcare office management systems and protocols.
- Ability to work flexible hours, including evenings and weekends.
- Occasional travel when required.
- Valid G Class Driver’s Licence and access to a reliable vehicle.
- Clear CPIC records check as a condition of employment.
- Knowledge, understanding, and experience of First Nation culture, values, and traditions.
